Introduction: Open fractures are one of the most common orthopedic emergencies, and their likelihood of complications increases with time of exposure. Objective: To investigate the initial management of open fractures based on recent scientific evidence. Method: A review of updated evidence-based literature was conducted, including guidelines and clinical studies, to offer systematic guidance on the initial management of open fractures. Results: Timely treatment combining debridement, early surgical stabilization, and a well-selected antibiotic regimen has proven to be key to improving patient prognosis. Conclusions: The Gustilo- Anderson system is essential for comprehensive management as it helps guide treatment to promote tissue recovery and prevent infection.
