Since 1949 Professor Takayama has adovocated that the causation of the so-called lumbago (low back pain) is directly or indirectly due to the presence of the distrubance of the transversal process. Taka-yama has conducted surgical treatment with remarkable success. Morphological studies were conducted on regio lumbosacralis in order to verify and otherwise elucidate the true state of lumbago. It was re-vealed that in most cases abnormalties of the ilio-lumbar ligament were present. The question that follows is whether the above abnormalties are related with lumbago. Further studies were made revealing answers to hitherto unexplained anatomical questions. The present report "on the structure of ilio-lumbar ligament" is the first of a series of studies for the elucidation of lumbago. 1) The variation of the ilio-lumbar ligament was divided into 6 groups and explained. 2) No structural abnormalties were seen in the ilio-lumbar ligament of the 1st, 2nd and 4th group. The structures of the 3rd ligament group varied from simple to complex and were further divided into 7 types and explained. 3) The 4th and 6th ligament groups were noted as ilio-lumbar ligaments in a narrow sense. 4) Explanations were made on the ilio-lumbar ligament of cases in which lumbosacral transitional vertebra was present. 5) 23.6% of all cases examined were formed to have symmetrical ilio-lumbar ligaments.
