Previous studies have aimed to define standard testicular elastography values in pediatric populations but are limited by small sample sizes or age ranges. Here, we report the largest normal testicular SWE evaluation cohort in pediatric patients under ten and ages 10 to 18. We aim to determine standard normal ranges in both pre-pubertal and pubertal patients. METHODS: Patients from one clinic location were screened for the IRB-approved study. Volunteers with no known history of testicular diseases, sex chromosome abnormalities, or testicular surgery were eligible. They underwent both testes' routine B-mode sonography and simultaneous multi-frame shear-wave elastography. Elastography measurements (kPa and m/s) were acquired at each testicle's superior, mid, and inferior poles. RESULTS: 178 testicles of 89 volunteers (mean age: 6.49±4.91, range: 2 months – 18 years) were measured. For analysis, participants were divided into two age groups: 2 months – 9 years (n=60) and 10 – 18 years (n=29). The average overall testicular elastography measurements significantly differed between the two age cohorts (4.73 kPa vs 4.02 kPa, p=0.03 and 1.22 m/s vs. 1.12 m/s, p=0.02). Additionally, there were statistically significant differences between the average elastography measurements of the mid poles (4.59 kPa vs. 3.69 kPa, p=0.02 and 1.21 m/s vs. 1.07 m/s, p=0.004) and lower poles (4.90 kPa vs 4.04 kPa, p=0.03 and 1.23 m/s vs. 1.14 m/s, p<0.05) in participants under ten compared to participants 10 and older. Table 1 reports the ranges of elastography measurements acquired in the two age cohorts. CONCLUSIONS: We established standard values of testicular stiffness in the largest cohort of normal pre-pubertal and pubertal pediatric patients to date. Prior work has investigated SWE's utility in evaluating pediatric testicular pathologies, such as undescended testes. Therefore, future work is needed to validate these measurements further to improve the utility of their clinical applications in evaluating childhood testicular disorders.
