In China, the gradual aging of the population, the high incidence of chronic diseases, and the increasing burden of healthcare have become widespread social concerns. The implementation of health management is a key link in improving people's health and controlling the total cost of medical care. This paper designs an intelligent health management system based on the Internet of Things, analyses the system architecture, system functions and system implementation process, and takes hypertension monitoring as an example. The system enables people to monitor their health status and obtain medical advice in a timely manner in their home life. The system achieves a shift from a hospital-centred medical model to a combined prevention and treatment model centred on family self-management, which is of great significance for the integrated prevention and treatment of chronic diseases.
