The information gathered in the assessment phase of the nursing process is used to try and make sense of the client's problem, prior to commencing therapy. This phase, termed problem analysis, represents a bridge between assessment and therapy. If the assessment has been successful, then the nurse is in a position to understand the problem and move forward. From this base, decisions can be made about an intervention. Indeed, such is the importance of problem analysis that it can also serve as therapy, as the client's bewilderment and worst fears can be eased by a plausible account of the problem.
