The content of the chapter is mostly based on the recent paper [13] which appeared in a special issue celebrating 50 years of the International Journal of Fracture. A number of corrections have been implemented, and another difference is that the color figures are now plotted with a bi-color scheme, to render them meaningful even when printed in black and white.
